Hello,
I'm in the Sunless Citadel; I defeated the Outcast and the little dragon dude in the tomb, but I haven't found any apples and I get the impression there is more to do here. Someone mentioned a trap door where the tree is, but there is NOTHING there. Anyone help me out??

After you defeat the Outcast, you would have finish your quest. Time to return to Oakhurst. Just go the way back you came from to Oakhurst to advance the story.

Just to further clarify, there are no apples to retrieve, it is all explained in the dialog.
If your PC is male, there are flowers down there that you can give to Chaarna later.
But thats about all of interest after that. Oh, except that ore Rurik can forge, and the two rings of the merchant woman's kids.
