I like using your extended palette entries to modify the colors of NPCs in the game but I think some of the better entries are after 200 and are lost to the random color palette. I've tried editing it but the colors come out degraded in some way. I was wondering what image editor is best for the task or am I stuck using the available format?

They're not lost to the random colour entries. You can use them on items, effects, and random colour table entries (ironically) and have them work just fine. I tried to arrange the colours in such way that plausible skin tones/etc. are kept <200.
If you want to customise your install, the download contains documentation on how to do so. You can use any tool that can edit/save .BMP files and edit 8-bit palettes.
