Some items are vanilla items, some are added by Undying (visor, mantle), there are a number of mods adding Balduran's xxxx to the game. You need to be more specific if interested in a special item.

But for reals, the very old Enhanced BG2 adds it for sure. Here's a possible link to download it from... but the mozilla firefox won't allow me to access it for stupid security reasons.
