Sorry, but for me the World Stone was destroyed, so they destroyed the whole world! But what of the people? Well, that's easy, they just go to another world/plane/realm. So the new game could be about the conquest of the new world, and corrupting everything by being evil and doing evil deeds...Warrior (Victor) -> Diablo
Rogue (Karena? I think) -> Blood Raven
Sorcerer (No clue) -> Wannabe Horizon
Also, breath out the Diablo fans. There will be a Diablo III, how will it then? Mephisto's Soulstone is destroyed, and so is Bhaals. Well that was planned from little Diablo. He freed himself from the soulstone by Victors help. And thereafter he helped Bhaal and Mephisto get lose from theirs. So when the soulstones were destroyed in Diablo II, it only unlocked the Prime Evils true powers. So prepare for world-dominating in Diablo III, since the Prime Evils have been running around with ~20% of their true powers in Diablo II, they are now at 100% again, and also immortal, they will just reincarnate all over. So prepare for Diablo MMM... even!

The world surely shattered in Harrogath, but not on any of the other known places. As AnTyrael said, it would take a hundred years to travel from Pandemonium Fortress to Harrogath, that is why they used the Red Little Portal.
Edit: Also while I am here (This is "abit" Off-Topic) but is it possible to raise stats permanently as it is in Diablo, but in an NPC, let's say, right before Tree of Life your charachter get so pumped up so her/his strength increased by two points? (You can answer in PM if you don't wanna spam )

Ah, but that's the thing, at the end of D2LoD Tyrael destroys the world stone, or what was left of it, the anchor that kept the world at bay, as it was corrupted by Baal. Global warming etc. etc. It is not my problem, it is 10 generations ahead so they will deal with "our" misstakes. And then in 100 generations the sun will warm the planet too much, so in 100 generations they better found out a way to counter the growth of the sun. In 100000000... generations the sun will die out, better they build a new one till then.
