Also, I thought it might be the fact that I gained a level, so I also bought the flail when there was no 'threat' of anyone in my party leveling up... but still no flail...
Is there any kind of minimum requirement to obtain the flail (must the PC or Will get to a certain level before it actually shows up?)

No, I am sure this is just a typo. Before the last version, several of us here were trying to come up with a good solution for how his flail made the poison bullets. I guess I changed the item's name and forgot to have the innkeeper's dialog adjust for this. I will add it to my list of bugs to check on.
